When you pre-order the game, you get the current pre-order bonus, and any ones that come out after you pre-order.

Yeah, just go in and tell them you want to. But you'll have to do it well in advance, going in the day beforehand won't do.

According to the Nuketown 2025 FAQ's on the Official Call of Duty website, Nuketown 2025 will be included in every launch copy of Black Ops II, so pre-ordering is the best way to ensure you get the bonus map.

Don't worry, just be sure that you pre-order the game to make sure you get Nuketown 2025. So in answer to your first question, yes.
