Make sure you have a tomahawk and ballistic knife, also handguns work too

1. Run with primary weapon and dive
2. RIGHT when you dive, switch guns
3. Right after you do 2, throw your tomahawk

Timing isn't as perfect as I made it seem but you don't want to take to long doing all these steps in the precious seconds you have while in mid-air
