Things you will need before doing anything:
Adobe Photoshop CS4 - Click You must login or register to view this content. to download the 30 day trial
Bankgothic MD BT font - Click You must login or register to view this content. to download it

Step 1: Install the Bankgothic MD BT font by going to Control Panel > Fonts
Then just drag and drop the font file you downloaded into that folder

Step 2: Open Adobe Photoshop CS4

Step 3: Click File > New
Set your settings as the ones below:

Step 4:
Fill the background colour black with the Paint Bucket Tool:

Step 5:
Click the Text Button and Write Whatever you want

Step 6:
Highlight it and change the colour to EEFFEE

Step 7: In the bottom right, right click the layer which has your text in it and click Blending Options
Then go to Outer Glow

Step 8: Click the colour button in the outer glow menu and change the colour to 7DFF6E click OK

Step 9: In the outer glow menu, Set size to 20

Step 10: Click File > New
Set your settings same as the ones below:

Step 11: Zoom into the new file 2000%

Step 12: Select black colour and paint a quare dot directly in the middle with the pencil tool, same as below:

Step 13: Click Edit > Define Pattern
Type in MW2 Font and click OK

Step 14:
Now go back to the old image by clicking the left tab in the top:

Step 15:
Click new layer in the bottom right:

Step 16: Select the pattern stomp tool, and make sure that the pattern you have just created is selected in the top:

Step 17: Lightly go over your text with the pattern stomp tool, do not hold it on one area for too long and make sure you cover the whole text

Step 18: In the bottom right, set opacity to 23%

Yeah, theres 1 or 2 vids on youtube but they don't explain it clearly, and they don't have the colour hex codes
So I figured I'd just post it up here in a video-less form, because it makes it easier to follow

You have to select the layer with the text on it
Which looks the layer below the one you have selected in the picture

If you still can't select blending options, try rasterizing the layer
