Seems like someone knows how to lag switch. Here's a 'theory'...
I'm pretty sure the guy who 'made' it did it like this:
Private Match> Custom Game> Edit Default Classes > Select Primary > Lag Internet / Lag Switch? > Quickly Go back into equipment menu and select throwing knife > Get internet back online quickly > Enjoy your primary which is a throwing knife now. (possibly)
That's my guess...but
the only other way would be to hack a split-screen profile with a gamemode (if there even is such a thing)
So my guess is he lag switched the menus to get the things. I bet there's other ways but I don't care at the moment.
