Possibly bad file downloads and not everything downloaded or it could simply be your EBOOT.bin.

Try swap one out for a ANTIBAN EBOOT you can make your own using BO2 EBOOT Makers Smile

Try find a new BO2 and re-download it if the above does not work!

Any game? As in you can get it to a game lobby before freeze or just can't boot the game from mm?
