Go to ps3 sound settings and make sure multi output is selected. Also rebuild your database and change hdmi port.

Its not always its rare like once a week any idea what it could be?

If it happens when you are playing bo2. They didnt complete the sound for ps3 bo2 and after a while of playing it glitches and sounds like a helicopter. Just go to zombies and then back to multiplayer and you will be fine.

Does it make a loud kind of buzzing/screeching sound? Like the audio seems strechted?

Yes, all you have to do is either just quit game and then go back on or just go to zombies. PS3 BO2 the sound track was never completed.
