Make sure your HDMI cable is well connected and not harmed.

are you jailbroken or on OFW? not sure but this might have something to do with it

as said above about the HDMI cable is this the first time using that tv your connecting it to, or first time with that cable... cheap cables or cheap tvs could more then likely cause issues similar to that. same with damaged ports or cables..
