If you are on OFW, run the game offline, the error may be profile card related, go to recovery mode, choose 'Restore System Files' and 'Rebuild Database'. Then clean disc, Delete all related data from the Game Data Utility, re-install the game, patch, and any DLC.
If on CFW, reinstall cfw and make sure latest spoof is installed, go to multiman 'fix file permission'. Or create PS3ISO (some games need to be run as disc format).
