Do a barrel roll!
"Continue" from Main Menu then do the little Quicktime Event and then make the choice "Kiss North."
Then use the handprint machine with Connor.
Pops after credits at the end flowchart screen.
There are more end game trophies this save will pop if you don't have them as well:
"Escape Death"
"Moral Victory"
"Undefeated"
"Partners"
"Happy Family"
"This Is My Story"
"An Army Of Me"
Download save for "Survivors" trophy and extras listed above:

For Bookworm trophy:
Pick Chapters from the Main Menu.
Select Chapter 3 - Shades of Color
Press Triangle to Load Chapter.
Go ahead and say "Save" just to make sure.
Right at the beginning of the chapter on the park bench on your right is the last magazine needed.
Most magazines have 2 covers, including this magazine.
Scroll through all the pages on each cover.
"No. 10 – Tech Addict: Secrets of Androids" & "No. 19 – Green Earth: Climate Change-Up"
Complete this super short chapter and the trophy should pop.

"I'll Be Back" trophy:
Continue from Main Menu.
Go in the house and talk to Hank, this will unlock at the chapter end flow chart.
(After this chapter this is a 'Platinum Save'
**You can also use this save for the "Just A Machine" trophy**
From the Main Menu Chapter Select "The Bridge" chapter.
Once you have the options select - cold, cold, not alive, nothing.
Hank will shoot Connor and the trophy will pop at the chapter end flow chart.

As long as this is the same CUSA as your own copy of the game, check out the very first section in the below link on "resigning saves". If you continue to have issues, the more details you can provide the better we can help.
