Do a barrel roll!
The save loads you into a room. An open cage in this room contains the 43rd Lorestone. After that, in this same room, to the right of a locked wooden door is a hole in the wall. Go through the hole and through the tunnel. Then proceed up the spiral staircase all the way to the top where you will find the 44th and final Lorestone. Pop that trophy.

This is the only trophy that requires you to do anything other than the story. With this and just playing through the story you will achieve the Platinum.
