How the issue occurred: No clue. Happened before and its happening again.
I have tried to fix my issue with: Everything here. New account, reinstall, rebuild database, restore filesystem, change eboots, selfs, load from mm, load from webman, load from disc, and reinstall CFW.

hmmm this happend to me today after loading the game after months of not playing. so also no idea.. must be a patch or something..
