glitch warning, after sequence 5 mission 2 is 100% sync and trophy does not pop, replay sequence 1 mission 1 and finish it to correct the glitch, trophy will pop after that

tips for 100% sync in mission 2 sequence 5;
take out two guards on roof with poison darts, watch cutscene, immediately run to the right and run towards the the very end of the left edge of the fort and take out the guards with poison darts as you go along, as soon as he appears in the corner and no guards are present shoot the barrel and quickly drop down to loot his body and then jump back up where you came and swing over with the whip and escape as fast as you can before you are discovered, good luck.

If I recall, several of them are unmissable story-based spawns, and others are in fixed points around the map. If you check at what point in the game you are, and cross-reference that against a trophy/achievement guide, you could check the points where they spawn on the map as "collectibles." My guess would be that this save was created with one or more of those left, rather than a story-based one left.
