Could be faulty when updating try updating in Recovery mode

First downgrade to 3.55 with Rogero Downgrader then update at what you want

You need to install 3.55 OFW or 3.55kmeaw from Rogero or Rebug downgrader firmwares. After that install any CFW of your choosing

Which DEX Firmware are you on? You said 4.78, but is it Rebug, Ferrox, etc.

Is your usb flash drive formatted to fat32?
