-downgrading to 3.55 DEX and istalling PS3ita 4.21 DEX
-restoring Ps3
-reflashing to DEX
-tried changing wired to wireless and wireless to wired
-new console id

Dude the same thing happened to me, I got the following by way recovery and formatted and returned to normal

By format and recovery, you mean you went into recovery mode and reformatted the hard drive?
