Do a barrel roll!
I have two save files for Outlast the game and for Outlast Whistleblower the DLC and they are CUSA-07742.
Save file number one is for Oulast at the last checkpoint in the underground Lab chapterand it's only missing Note 31 which is Death of Hope. The Timestamp for the last collectible is 4:16 in the video.

Save file number two is for Outlast Whistleblower at the checkpoint in the Exit chapter and it's only missing Document 13 (Three Blind Dreamers) at 8:53 and
Note 18 (Jeremy Blaire) at 9:16 in the video.
