Save is at The Epilogue, trigger the cutscene and the trophies will pop at main menu. This save will pop both trophies for Grounded and Grounded Mode Plus.

As of May 14th 2014, i change the param PFD, and usr data from my save into the hacked save, loads fine, but when I load game it starts from the beginning of the game.

Quit out to main menu and go to chapters > epilogue, and also try swapping out the param.sfo as well.

You place your param.sfo and pfd into my save. Theoretically it should work both ways, so long as save slot numbers match up.
